Sadly, the most likely explanation was that DMP4 was returned to Enterprises instead of the Film Library, and Nash's order to "In-Ci-Ne-Rate-All-Film-Prints" saw it destroyed.

I worked With Justin Smith for many years and we talked about this severa times, as richard pointed out, if a film was not returned on time then stern memmos were issued until it was, and Justin was old school in that he was very precise and proper about returning tapes / films here labeling everything correctly and doing everything by the book. Howeaver this film was lost it was AFTER Blue peter had the film.

Every item has been individually checked during the Archives massive transfer from Windmill Road to their new premises. Suffice to say, it's not improperly cataloged.

Also,

The print of Dalek masterplan Ep:4 was never returned to the library after blue peter borrowed it. I still have a printout from my time at the library showing it is (or certainly was - in case they have changed the records)still massively overdue and was last on loan and not returned from it's loan.
If Justin was as precise as he says he recalls he was, he slipped up over Masterplan 4. It's still booked out to BP!
Hope that helps,
Davidx

That's a print-out I'd like to see. :3 Well, it certainly sounds like someone intercepted it as it traveled from Blue Peter back to the library. How would that work exactly? Did people walk films back to the library or were they sent via mail? I'm assuming it operated much like any library and people walked in to check in and out various materials.

Heres a question. If blue peter didnt borrow tenth planet 4, would it have existed today or would it have been junked and we wouldnt have the clip.

We'd still have the regeneration. It's on the 8mm Australian cine clips.

On the topic of tenth planet episode 4, I have always been more interested to see power of the daleks episode 1, to see a bit more of Troughton's first episode as the doctor; to be frank it must have seemed a fairly risky thing to completely replace the lead actor for a popular show in such a way. I would be very interested to see Troughton warming up in the role.

Experimental, yes; but not that much of a risk. A change in lead actor happened all the time in long-running stage productions, radio serials, movie series, and TV drama in Britain, so a new actor taking over in DW would not have bothered audiences that much.

Of course, Doctor Who was the first time such a change had actually been incorporated into the narrative structure of the show. The producers could quite easily have had Troughton be the Doctor without any explanation or comment; Ben and Polly (and the audience) would just accept it without even mentioning the new face!

Thankfully, they did introduce the concept of "rejuvenation/regeneration" as that contributed towards and helped secure DW's ongoing success.
